The Rules
1.
This “Request Information Form” must have all required
fields filled out to be considered a “valid entry”.
If you don’t know an answer, please type “unsure” in
that field.
2.
Event location must be in Columbus or Central Ohio and travel
charges will apply for events considered of “out-of-town”
3.
Only one contestant per event is allowed and multiple entries
will be rejected and considered invalid. (Example if your event
is a wedding reception and both the bride AND groom enter, both
entries will be rejected)
4.
We will not sell your Email or any contact information to
any 3rd party however, we will contact you by phone or email to
announce
the contest winner and to be able to follow up with
you or validate he legitimacy of “Request Information Forms”
5.
If you are already a booked customer and win, you will be refunded
any money already paid to us.
6.
Your odds of winning will be determined by the number of entries
at the end of this contest (March 31 2009)
7.
If you win, you must immediately scream to all you friends
that “I
